How to check your health record in Bulgaria (NHIF and NHIS)

How to check your health record in Bulgaria (NHIF and NHIS)

Pravko28 October 202610 min

An ЕГН is not enough. The NHIF record opens with a ПИК, УКД or e-signature; the NHIS (my.his.bg, еЗдраве) only with a qualified signature or paired phone.

Do you want to see which check-ups, prescriptions and tests are recorded in your name, or do you suspect that someone has billed the health insurance fund for an appointment you never had? Bulgaria has two health records, kept by two different institutions, and many people log in to one of them and decide that “nothing shows up”. Here is which is which, what you log in with, and what to do if you see something that is not yours.

❓How do I check my health record by ЕГН?

✅The ЕГН (Bulgarian personal identification number) alone does not open a record. The record at the National Health Insurance Fund (NHIF, НЗОК), showing what has been reported and paid for you, opens at nhif.bg/bg/pis with a personal identification code (ПИК) from the National Revenue Agency (NRA, НАП), with a unique access code (УКД) from the fund, or with an electronic signature. The electronic health record with check-ups, prescriptions, referrals and results is in the National Health Information System (NHIS, НЗИС), at my.his.bg and in the еЗдраве (eHealth) app. There an NRA ПИК does not work: you need a qualified electronic signature or a one-off pairing of your phone in person at a Regional Health Insurance Fund office (РЗОК), a Regional Health Inspectorate (РЗИ) or “Информационно обслужване” (Information Services).

Two records, not one

The first record is that of the NHIF. It shows the medical and dental activities that doctors, dentists and hospitals have reported and the fund has paid for you. The fund calls it “Review of the record of the health-insured persons” (“Преглед на досие от здравноосигурените лица”) and it sits in its Personalised Information System (PIS, ПИС). The right to receive this information is set out in Art. 64 of the Health Insurance Act.

The second is the electronic health record in the NHIS, which under Art. 28г of the Health Act is administered and maintained by the Ministry of Health. It holds the electronic health documents: e-check-ups, e-prescriptions, e-referrals, test results, vaccines, regardless of whether the fund paid or you did. You log in through the portal my.his.bg or the mobile app еЗдраве.

Checking with an NRA ПИК: the NHIF record step by step

If you already have a personal identification code (ПИК) from the NRA, this is the fastest route to the NHIF record. You do not need to visit an office and you do not need an electronic signature.

Logging in to the NHIF record with an NRA

1

The address is nhif.bg/bg/pis. The e-service “Review of the record of the health-insured persons” (“Преглед на досие от здравноосигурените лица”) from the “E-services” (“Е-услуги”) menu on nhif.bg leads there too.

2

The button takes you to the login of the NRA portal. The other two buttons are “Login with digital certificate” (“Вход с цифров сертификат”, an electronic signature) and “Login with Unique Access Code” (“Вход с Уникален Код за Достъп”).

3

Enter the identifier and the ПИК

Choose the type of identifier (ЕГН, ЛНЧ, the personal number of a foreigner, or a service number) and enter the ПИК. If the portal asks for a check against bots, confirm it.

4

Open “Patient information” (“Пациентска информация”)

According to the fund, this menu describes all the medical and dental activities reported and paid for you by the NHIF.

5

Check your children too

On the right you enter the ЕГН of a child for whom you chose the personal doctor (GP), and you see what has been reported for them.

Three ways to log in to the fund’s record

Choose one of the options

1

NRA ПИК

A free code, issued at an NRA office. If you already use it to check your contributions or taxes, it works here too.

or
2

NHIF УКД

A free code, issued in person at any RHIF office in 3 to 5 minutes. Only for the compulsorily health-insured, including those with interrupted rights.

or
3

Qualified electronic signature (КЕП)

Issued by a trust service provider, on a card or as a cloud signature. Of the three options, only it also opens the NHIS.

An NRA ПИК does not open the NHIS

Many people log in with an NRA ПИК, do not see their prescriptions and tests, and decide that the record is empty. It is not: you are looking at the fund’s record, not the electronic health record in the NHIS. According to the NHIS questions and answers the NRA ПИК, the ПИК of the National Social Security Institute (NSSI, НОИ) and the NHIF УКД give a “low” level of assurance under Regulation (EU) No 910/2014, so the NHIS does not accept them for logging in to the electronic record. As of 28 October 2026 the “Login with eAuthentication” (“Вход с еАвтентикация”) button on my.his.bg offers only a qualified electronic signature: on a smart card or USB, or a cloud one (B-Trust, Evrotrust, InfoNotary). The NHIS announces an upcoming integration with the electronic identification app for Bulgarian citizens, but for now logging in with it is not possible.

УКД: the free code from the fund

The unique access code (УКД) is the alternative to the ПИК for people who do not have a code from the NRA. According to the NHIF page on the УКД and an answer from the fund of 23 March 2022, the code is issued free of charge at any regional health insurance fund office (РЗОК), wherever you live, in 3 to 5 minutes. You submit an application in person against an identity document and receive the code on paper in a sealed envelope, also in person, against a receipt. Only the compulsorily health-insured are entitled to a УКД, including when their rights are interrupted. Under the fund’s internal rules on the УКД an attorney can also submit the application and receive the code, but only with an express power of attorney with a notarised signature.

What a УКД gives you, according to the fund

Listed on the official NHIF page

Access to your record at the fund and a check of every reported visit

If you wish, an email notice for every check-up, dental activity or hospital treatment in your name

Access to the record of your children under 18 too, if you chose their personal doctor

A basis for a report to the NHIF or the РЗОК if the record does not match the care actually received

Ask Pravko and get an answer with citations from the law.

NHIS and еЗдраве: check-ups, prescriptions, tests

The NHIS receives the e-prescriptions the doctor issues, the medicines the pharmacy dispenses and the results the laboratory uploads. According to the еЗдраве page and the NHIS questions and answers, in the record you see:

What is in the electronic health record

Check-ups: outpatient data from medical examinations carried out

Dental check-ups: from 6 October 2026 a separate module with dental status and treatments carried out

Referrals: issued e-referrals for tests or for a specialist

Results of medical-diagnostic tests

Vaccines: routine, seasonal and emergency

Prescriptions: e-prescriptions and information on taking medicines

Hospital stays and medical expertise (for TELK, the expert medical commission: validity and diagnosis, without the decision itself as a PDF)

The dental module is described in an NHIS announcement of 6 October 2026; to see it in еЗдраве the app must be updated. Logging in to the record works in two ways. On a computer you open my.his.bg and identify yourself with a qualified electronic signature. On a phone you use the еЗдраве app, which you first have to “pair” with your record. Here is the procedure described by the NHIS:

Pairing еЗдраве with your record

Download

Free from the App Store or Google Play.

With a qualified electronic signature on a computer

Log in to my.his.bg with your signature. From the menu choose “Profile” (“Профил”), “Device management” (“Управление на устройства”) and “Add new device” (“Добави ново устройство”). A security QR code appears on the screen, which you scan with еЗдраве.

With a cloud signature on the phone

If the cloud signature is installed on the same phone, pairing is done directly in the app by following the registration instructions.

Without a signature: in person

Go to an office of the РЗИ, the РЗОК or “Информационно обслужване” (Information Services) with an identity document. An employee generates a one-time security QR code, which you scan with еЗдраве.

Protection and notifications

Finally you choose whether to use additional biometric protection and whether to receive notifications. If you deactivate the app, your personal and medical data are deleted from the phone.

Access to your own record is free of charge. This is guaranteed by Art. 28д of the Health Act.

Health Act (ЗЗ)

Art. 28д(1), item 1

Your access to your own record in the NHIS is free of charge by law.

Official Bulgarian text

„Право на безвъзмезден достъп до Националната здравноинформационна система имат: 1. гражданинът – до информацията в неговия електронен здравен запис;“

Check-ups and tests you paid for should be in the record too

A common question to the NHIS is why a paid check-up or test, covered by a private health fund, does not show in the record. The system’s answer is clear: since 8 October 2024 Art. 28г(3а) and (3б) of the Health Act oblige medical and non-medical specialists in medical and health establishments to create an electronic health record for every activity, regardless of whether you are health-insured and who paid. If you do not see a paid check-up, the NHIS advises you to remind the doctor or the laboratory.

Health Act (ЗЗ)

Art. 28г(3а)

The obligation is for every activity carried out, with no exception for paid ones.

Official Bulgarian text

Source of the data: his.bg and an NHIS announcement of 3 September 2026.

You saw a check-up you never had: what to do

This is the most important reason to check your record. If the fund’s record shows an activity you did not receive, the fund may have paid the provider for something that was not carried out. You have two separate routes and you can use both: a report in the app and a complaint under the Health Insurance Act.

Reporting someone else’s check-up in your record

1

From 1 August 2026 the “Public control” (“Публичен контрол”) module in the app shows the reported activity (type, number, doctor, date, diagnoses). If you do not recognise it, you submit a report with two taps, enter a phone number and an email for contact, and confirm it. The report is registered automatically and sent to a control body: the РЗИ, the РЗОК or the Medical Supervision Executive Agency (Изпълнителна агенция „Медицински надзор“). The result of the check comes in the app.

2

Complaint to the director of the РЗОК

In writing, to the regional health insurance fund office. The fund also accepts written reports of irregularities at its head office: NHIF, 1407 Sofia, 1 Krichim Street (ул. „Кричим“ № 1). Describe what you see in the record and that you did not receive this care. The law expressly lists “a reported but not carried out medical activity” (“отчетена, но неизвършена медицинска дейност”) as a ground for a complaint.

3

Keep evidence

A screenshot of the record showing the date, the doctor and the type of activity. That way the inspectors find the entry more easily.

The right to complain is in Art. 35(2) of the Health Insurance Act and is given to compulsorily insured persons.

Health Insurance Act (ЗЗО)

Art. 35(2)

The right to complain to the director of the РЗОК is yours as an insured person.

Official Bulgarian text

„Задължително осигурените лица имат право да подават жалби пред директора на съответната РЗОК, когато не са удовлетворени от медицинските дейности, свързани с оказаната медицинска помощ. Жалбата се подава по реда на глава втора, раздел X, като в нея се описват причините и се посочва най-малко едно от следните основания:“

Health Insurance Act (ЗЗО)

Art. 35(2), item 1

The first listed ground for a complaint is exactly someone else’s check-up in your record.

Official Bulgarian text

Not a trifle

In the module’s first month citizens submitted 1,760 reports, and the fund’s control bodies opened 560 checks, according to the NHIS. Most reports are about check-ups, results of referrals and hospitalisations. For events in a linked child’s profile the report is submitted by the parent.

The record of a child and of a relative

A child under 18. In the fund’s record, access belongs to the parent who chose the child’s personal doctor: you enter the child’s ЕГН in the field on the right. After the age of 18 the parent’s access to the child’s data stops automatically, and the adult can ask for their own УКД in the usual way (Art. 8 of the NHIF’s internal rules on the УКД). In the NHIS you log in to your own record and from the menu with your initials in the top right corner you choose the child’s name.

Another adult. Health data is personal data and nobody can look at your record instead of you without a legal basis. Art. 28б of the Health Act lets you authorise a person in writing to read your medical documents and make copies. This power of attorney is presented at the medical establishment. In еЗдраве there is for now no login for another adult: relatives’ access to the records of adults and the seriously ill is in the NHIS development plan for 2026. Medical establishments, the fund, insurers, state bodies and professional organisations get access to your NHIS record only after express written consent, under conditions and in a procedure set by an ordinance. From 1 November 2026 a law promulgated in the State Gazette (Държавен вестник), issue 68 of 2026, adds to them the medical commissions at the branches of the NSSI (when examining a person in a pension procedure), also only with express written consent.

Health Act (ЗЗ)

Art. 28б(2)

The power of attorney must be in writing. A spoken “let my wife ask” is not access to the documents.

Official Bulgarian text

„Пациентът има право да упълномощи писмено друго лице да се запознае с медицинските му документи, както и да направи копия от тях.“

Health Act (ЗЗ)

Art. 28д(2)

Medical establishments, the fund, insurers, state bodies and professional organisations see your record only with your express written consent.

Official Bulgarian text

„Предоставянето на достъп до информацията в електронния здравен запис на гражданина на лицата по ал. 1, т. 2 – 5 се извършва само след изрично писмено съгласие при условия и по ред, определени в наредбата по чл. 28г, ал. 6 при спазване на изискванията на чл. 28г, ал. 8.“

Sharing with a doctor of your choice from the app

From April 2026 еЗдраве and my.his.bg have an “Access” (“Достъп”) module: you choose a specific doctor from a list with names, specialty and UIN (УИН) and give them access to your record, or you approve a request sent by the doctor. For children’s profiles, access is granted after confirmation by a parent, according to the NHIS.

A deceased relative. Heirs and relatives in the direct line and in the collateral line up to the fourth degree inclusive are entitled to see the health information about the deceased and to make copies of their medical documents. There is no function in еЗдраве for heirs’ access, so this right is exercised at the medical establishment that keeps the documents.

Health Act (ЗЗ)

Art. 28б(3)

The right of heirs and relatives goes up to the fourth degree in the direct and collateral line.

Official Bulgarian text

„При смърт на пациента неговите наследници и роднини по права и по съребрена линия до четвърта степен включително имат право да се запознаят със здравната информация за починалия, както и да направят копия от медицинските му документи.“

Paper or screen: which counts

More and more documents appear only electronically: medical notes for school, prescriptions, referrals. Nobody can require you to bring a printout of such a document, except for technical necessity. But the reverse also applies: if you want the information on paper, you have the right to get it.

Health Act (ЗЗ)

Art. 28г(9)

Paper may only be required for technical necessity or if you wish.

Official Bulgarian text

„Не може да се въвеждат изисквания за възпроизвеждане или съхранение на хартиен носител на електронни документи, които са част от електронните здравни записи в Националната здравноинформационна система, освен при обективна необходимост по технически причини или когато пациентът изрази желание да получи информация на хартиен носител.“

Health Act (ЗЗ)

Art. 28е(2)

Where a paper document and the NHIS record disagree, the electronic record prevails as a rule.

Official Bulgarian text

„При противоречие между документ на хартиен носител и електронен здравен запис относно здравната информация в Националната здравноинформационна система по отношение на здравната информация предимство има записът в Националната здравноинформационна система, освен ако няма обективни причини, които поставят под съмнение автентичността на електронния документ, или ако електронният документ е електронен образ на документ на хартиен носител.“

Separately from the online records, you have the right to receive from the medical establishment the health information about yourself, including copies of your medical documents. If a hospital or a doctor who works with the fund refuses you, refused access to medical documentation is a separate ground for a complaint to the director of the РЗОК. (A complaint under the Health Insurance Act concerns care provided under a contract with the fund; for a fully private check-up the РЗОК is as a rule not competent.)

Health Act (ЗЗ)

Art. 28б(1)

Copies of your own medical documents are your right, not a service at the doctor’s discretion.

Official Bulgarian text

„Пациентът има право да получи от лечебното заведение здравната информация, отнасяща се до неговото здравословно състояние, включително копия от медицинските си документи.“

Health Insurance Act (ЗЗО)

Art. 35(2), item 3

The third ground for a complaint to the РЗОК is refused access to medical documentation.

How far back you can look

The fund is required to give you information on the medical and dental care you used and its price for the last five years, and under Art. 67 of the Health Insurance Act your data is kept at the NHIF for five years after your health insurance ends. The electronic record in the NHIS, according to the page on its modules, has been available to citizens since 2020, at first with prescriptions and vaccines. According to the NHIS development plan the e-check-up module was created in 2021 and the еЗдраве app was launched in 2022. The law also requires back-filling. The October 2024 amendments to the Health Act obliged the fund to send its data from 1 January 2010 onwards to populate the records (the period was one year, until October 2025). Medical establishments, for their part, must by 1 January 2027 enter retroactively their activities after 1 January 2022 for which they have no record but do have structured electronic data. Documents that exist only on paper are not covered by this obligation. That is why older records may appear gradually, and their absence is not in itself a mistake.

Act Amending and Supplementing the Health Act (State Gazette (ДВ) issue 85 of 2024)

§ 15 of the Transitional and Final Provisions

The fund’s data from 2010 onwards must populate the electronic records in the NHIS.

Frequently asked questions

Can I check my health record with only my ЕГН, without a code?

No. With only the ЕГН (and a code from a picture, a captcha) the fund shows who your personal doctor is, but not the record. The record contains sensitive data and opens only with an NRA ПИК, a УКД or an electronic signature (for the fund), or with a qualified electronic signature or a paired phone (for the NHIS). Officially it opens at nhif.bg, at my.his.bg and in the еЗдраве app. Do not enter an ЕГН, ПИК or УКД on other sites that promise such a check.

Is the “Health record from Generali” the state record?

No. That is a private app linked to the insurer Generali (in it the platform is called “My Health Record”, “Моето Здравно Досие”). In it you upload documents and results yourself and submit claims for compensation. The state records are two: the NHIF’s at nhif.bg and the NHIS’s at my.his.bg and in the еЗдраве app.

Does it cost anything?

Not for the access itself. The ПИК and the УКД are issued free of charge, access to your own record in the NHIS is free by law, and medical establishments have no right to charge you for entering your data in the system. The only possible cost is the electronic signature, if you decide to get one: the price and terms depend on the provider.

I see my TELK decision only as a diagnosis and a deadline. Where is the document itself?

In еЗдраве the validity and the diagnosis are shown, but not a PDF of the decision. For the document itself the NHIS refers you to the electronic system “Medical Expertise” (“Медицинска експертиза”). We have described how to check a TELK decision online in a separate article.

My record at the fund is empty. Does that mean I have no health rights?

Not necessarily. An empty record usually means that the fund has not paid for activities in your name, for example because you only go to paid check-ups. Whether your contributions are in order is checked separately, in the health insurance status check. The fund gives a УКД to people with interrupted rights as well, so the record can be seen then too.

I changed my personal doctor, but the NHIS still shows the old one. How do I fix it?

You cannot do it yourself. According to the NHIS the information about the personal doctor comes from the fund and is updated automatically after the doctor submits the data. For when exactly the change takes effect and what is submitted, see the article on checking and changing a personal doctor.
